Morning Cortisol Saliva Test: What It Measures and How It Can Guide Your Next Steps

Waking unrefreshed or struggling to become alert can make the start of the day difficult. Sleep, medicines, daily routines and medical conditions can all influence morning energy. A morning cortisol saliva test provides one piece of information that may help a healthcare professional investigate these concerns.

The test measures cortisol in a single morning saliva sample. Its value lies in adding an objective measurement to a broader assessment and helping you decide what to investigate next.

What Is Cortisol?

Cortisol is a hormone produced by the adrenal glands, which sit above the kidneys. Its production is regulated through communication between the brain and adrenal glands, often called the hypothalamic–pituitary–adrenal, or HPA, axis.

Cortisol participates in the stress response and interacts with several other body systems. Its concentration changes over the day, so collection timing matters when interpreting a result.

What Does the Morning Cortisol Test Measure?

The Male Morning Cortisol Saliva Test and Female Morning Cortisol Saliva Test measure one marker: morning free cortisol, using liquid chromatography–tandem mass spectrometry (LC-MS/MS).

Free cortisol is the unbound fraction of the hormone. ZRT explains that saliva reflects free cortisol rather than the cortisol attached to binding proteins in blood. Saliva also allows convenient collection without a blood draw.

Both product options measure the same marker. They do not measure testosterone, oestradiol, progesterone, thyroid hormones or DHEAS. Those require separate tests or a broader panel.

Why Is Morning Collection Important?

Cortisol follows a daily rhythm and normally increases around waking before declining later in the day. A morning measurement therefore needs to be interpreted in relation to when you woke and when you collected.

Recording the actual collection time helps the laboratory and your healthcare professional interpret the finding. If you work night shifts or have an unusual sleep schedule, ask for collection advice before testing.

Is This a Cortisol Awakening Response Test?

A single morning cortisol measurement is different from a cortisol awakening response assessment.

The cortisol awakening response, commonly called CAR, describes the change in cortisol after waking. ZRT’s six-sample CAR approach includes collections within five minutes of waking, at 30 minutes and at 60 minutes, followed by later samples.

One sample cannot show how much cortisol increased between waking and the later morning measurements. It also cannot show your afternoon or bedtime levels.

Test Information provided
Single morning cortisol Cortisol concentration in one morning sample
Four-point cortisol profile Measurements at several points across the day
Cortisol awakening response assessment Changes after waking, with later sampling according to the panel

ZRT offers these as distinct testing options. The appropriate choice depends on the question you and your practitioner want to investigate.

Who Might Consider a Morning Cortisol Measurement?

This test may be a starting point for people seeking information to discuss with a healthcare professional when experiencing:

  • Persistent morning tiredness or low energy.
  • Difficulty becoming alert after waking.
  • Waking unrefreshed.
  • Feeling stressed, tense or overwhelmed.

These concerns are listed on the Hormone Lab UK product pages. They are not specific to a cortisol disorder, and having them does not automatically mean your cortisol is abnormal.

How Should You Collect Your Sample?

ZRT’s current single-tube instructions say to begin collection within 30 minutes of waking. Before collection, do not eat, drink anything except water or brush your teeth.

Wash and dry your hands, rinse your mouth with water and wait five minutes before collecting. Fill the tube at least halfway, excluding bubbles, then secure the cap and label it with your name, collection date and time.

Complete the paperwork and follow the return instructions. ZRT advises freezing samples that will not be shipped within 24 hours.

Hormone Lab UK additionally lists collection before 9am, ideally between 6am and 8am. Follow the instructions supplied with your kit and clarify timing if your waking schedule does not fit that window.

Record medicines and hormone treatments on the requisition form. If you use steroid medicines, ask your healthcare professional about appropriate collection timing. Do not stop prescribed treatment independently.

What Can Your Result Tell You?

Your report shows your morning cortisol measurement alongside the applicable laboratory reference range and specialist commentary.

A result outside the range can prompt further investigation, while a result within range provides information about that particular sample. Neither outcome, by itself, explains every symptom.

Interpretation should take account of collection timing, sleep, medicines and your clinical history. ZRT notes that reference ranges depend on the laboratory and testing method, so use the range on your own report rather than comparing the number with unrelated online ranges.

A single morning saliva result does not independently diagnose conditions such as adrenal insufficiency or Cushing’s syndrome. Further clinical assessment and appropriately selected investigations are needed when these are suspected.

What Happens Next?

Your healthcare professional can use the report to decide whether a daily cortisol profile, CAR assessment or other investigations would be useful.

Hormone Lab UK states that results are available within 3–5 working days after laboratory receipt; postal transit time is additional. [2, 3]
