Morning Cortisol Saliva Test for Women
A convenient at-home measurement to support a personalised review of morning tiredness and stress-related concerns.
If you regularly wake feeling exhausted or struggle to become alert, a morning cortisol result provides additional information to discuss with your healthcare professional. Your report includes your measured level, a laboratory reference range and laboratory comments to help explain the finding.
What Does This Test Measure?
Morning free cortisol — 1 marker, measured in saliva using LC-MS/MS.
Cortisol participates in the stress response, energy regulation and your sleep–wake cycle. This test measures the free, unbound cortisol in your saliva at the time of collection.
Collect one morning sample according to the instructions supplied with your kit. A single measurement provides a morning snapshot; a cortisol awakening response assessment uses several samples collected after waking.
Who Should Consider This Test?
Women seeking a morning cortisol measurement to review with their practitioner, particularly when experiencing:
- Persistent morning tiredness or low energy
- Difficulty becoming alert after waking
- Waking unrefreshed
- Feeling stressed, tense or overwhelmed
How Can Your Results Help?
Add Information to Your Assessment
Your result shows whether your morning cortisol measurement falls within the laboratory’s reference range. Reviewed alongside your symptoms, collection time, sleep and medication history, it helps your practitioner decide whether the finding needs further investigation.
Support a Review of Your Daily Routine
Your report can provide a starting point for discussing sleep consistency, caffeine use, exercise intensity and recovery time. Your practitioner can help you choose practical changes suited to your circumstances and agree how to track improvements in energy and wellbeing.
Help You Choose the Next Investigation
If symptoms continue, your practitioner can consider whether a daily cortisol profile, cortisol awakening response assessment or other tests would provide more useful information.
For women with menstrual changes, PMS or menopausal symptoms, this measurement can contribute to a broader assessment. Your practitioner may recommend separate reproductive-hormone, thyroid or nutritional investigations according to your symptoms.

How the Report Comments Help
The laboratory comments explain your result in the context of the information provided on your requisition form. Recording your symptoms, collection time, medicines, supplements and hormone treatments helps make that interpretation more relevant.
These comments give you and your practitioner information to discuss—not simply a number to interpret alone. Laboratory interpretation is provided by laboratory specialists and is separate from a medical consultation.
What Should You Do After Receiving Your Results?
- Read your result and laboratory comments. Note any findings highlighted for review.
- Share the report with your healthcare professional. Include your symptom history and current treatments.
- Agree your next steps. These may include practical lifestyle changes, further investigations or treatment following clinical assessment.
- Track how you feel. Record morning energy, sleep and daily symptoms to help evaluate your plan.

Frequently Asked Questions
Why is morning cortisol important for women?
Morning cortisol directly suppresses progesterone via pregnenolone steal and impairs thyroid T3 conversion — making it one of the most common hidden drivers of PMS, cycle irregularity, fatigue, and hormonal imbalance in women. It is rarely assessed by standard GP panels.
When should I collect my sample during my menstrual cycle?
This test can be collected at any point in your cycle. Guidance on optimal timing is included in your kit instructions.
Do I need to fast before collecting?
No fasting is required. Collect your sample within 30 minutes of waking, before eating, drinking, or brushing your teeth.
Can I use this test if I am on HRT or the contraceptive pill?
Yes. This test is suitable for women on HRT, the contraceptive pill, or other hormonal medications and can help identify whether cortisol is contributing to residual or breakthrough symptoms.
Should I upgrade to the 4-point cortisol test instead?
If you suspect cortisol is disrupted throughout the day — particularly with afternoon energy crashes, evening anxiety, or night waking — the Female All Day Cortisol Test provides the complete daily cortisol rhythm. The morning test is the most appropriate starting point for women with primarily morning symptoms or as a first-step assessment.
Does the price include the laboratory fee?
Yes. The full laboratory analysis fee is included. There are no additional charges. You are only responsible for posting your sample to the laboratory.
How long is the kit valid for?
The kit must be used within 12 months of the purchase date.
How to Collect Your Saliva Sample
Collect one morning saliva sample at home using the tube supplied with your kit. No blood draw or clinic visit is required.
Choose Your Collection Day
For women testing estradiol, progesterone or testosterone, collection timing depends on your menstrual cycle:
| Your usual cycle length | Collection day |
|---|---|
| 23–26 days | Day 17, 18 or 19 |
| 27–30 days | Day 21, 22 or 23 |
| 31–34 days | Day 25, 26 or 27 |
| Shorter or longer regular cycles | 5–7 days before your next expected period |
| Infrequent or unpredictable cycles | Any non-bleeding day |
| No menstrual periods | Any day, following treatment-timing instructions where applicable |
Day 1 is the first day of menstrual flow, not spotting.
Prepare for Collection
- Three days before: Avoid anti-ageing or anti-wrinkle facial creams and biotin/vitamin B7 supplements, including hair and nail formulas.
- The night before: Stay normally hydrated without drinking excessive amounts.
- On collection morning: Do not eat, drink anything except water, or brush your teeth before collecting.
Record all medicines, supplements and hormone treatments on your Test Requisition Form.
Collect Your Sample
- Wash and dry your hands, rinse your mouth with water and wait five minutes.
- Begin collection within 30 minutes of waking.
- Allow saliva to collect naturally in your mouth, then transfer it into the tube.
- Fill the tube at least halfway, excluding bubbles. Collection usually takes 15–30 minutes.
- Secure the cap and label the tube with your name, collection date and time.
If You Use Hormone Treatment
Continue prescribed treatment and follow the treatment-specific timing instructions in the leaflet. For topical hormones, use gloves or an applicator for at least two days before collection to help prevent contamination. If you use steroids, troches or under-the-tongue hormone treatments, discuss collection suitability and timing with your healthcare professional.
Return Your Sample
Place the labelled tube and completed paperwork in the supplied kit packaging and follow the return-postage instructions. Freeze the sample if it will not be shipped within 24 hours.
